In the past, the likelihood of receiving exercise authorization from a physician had more to do with the physician’s personal views on exercise than the health status of the patient. (Hoffman, et al, 2016; Jamnik et al., 2011) For example, physicians who exercised as often as 4.2 hours/week were more likely to allow exercise prescription regardless of patient condition than were doctors who did not exercise regularly. The PAR-Q was developed in an effort to standardize activity screening for persons aged 15-69 years. (Chisolm, et al, 1975). The revised PAR-Q (r-PAR-Q) was developed to improve specificity of the PAR-Q by clarifying language. (Shephard, et al, 1981; Cardinal et. al. 1996) There are general screening questions which become more specific to each patient based on their answers. The PAR-Q+ is a 7-step questionnaire for use with persons of all ages. It screens for evidence of risk factors during moderate physical activity and reviews family history and disease severity. If a person answers yes to one or more questions, the individual should complete a thorough follow-up of medical questions and consult a physician about beginning physical activity. An interactive on-line version for medical follow-up questions is provided by the ePARmed-X+ (Jamnik, et al, 2011; Bredin, et al, 2013). If a consult with a physician is not recommended by the PAR-Q+, then the individual is cleared to participate in physical activity. (Jamnik, Gledhill, & Shepard, 2007; Joy & Pescatello, 2016)

Introduction: The Physical Activity Readiness Questionnaire (PAR-Q) and the Physical Activity Readiness Medical Evaluation (PARmed-X) are internationally renowned pre-participation screening tools. However, these forms were developed without evidence-based support. Moreover, feedback from end-users highlighted the need for refinement. Purpose: To examine the evidence-based support for the PAR-Q and PARmed-X and identify if further revisions were warranted. Methods: Ten systematic reviews were conducted to establish the exercise-related risks and effective risk stratification in healthy individuals (including pregnant women) and persons with prominent chronic medical conditions. This process adhered to the Appraisal of Guidelines for Research and Evaluation (AGREE) Instrument. Key Findings: Habitual physical activity is associated with a reduced risk for over 25 chronic conditions and premature mortality. Moderate intensity physical activity on most days of the week is of benefit for most patients with chronic conditions. The risks associated with a physically inactive lifestyle are markedly higher than the transient risks seen following acute exercise (in healthy and clinical populations). Changes to the PAR-Q and PARmed-X: The result of this process was the development of a new and enhanced pre-participation screening and risk stratification strategy that serves to reduce the barriers to physical activity for Canadians across the lifespan (including those with various chronic conditions). This included the development of the new PAR-Q+ and the online ePARmed-X+. The new screening tools and risk stratification strategy are now evidence-based, and serve to greatly reduce the barriers to physical activity for all (including those living with a chronic medical condition).

Background: The Physical Activity Readiness Questionnaire for Everyone (PAR-Q+) is the international standard for pre-participation risk stratification and screening. In order to provide a practical and valid screening tool to facilitate safe engagement in physical activity and fitness assessments for the Brazilian population, this study aimed to translate, culturally adapt, and verify the reproducibility of the evidence-based PAR-Q+ to the Brazilian Portuguese language.Method: Initially, the document was translated by two independent translators, before Brazilian experts in health and physical activity evaluated the translations and produced a common initial version. Next, two English native speakers, fluent in Brazilian Portuguese and accustomed to the local culture, back-translated the questionnaire. These back translations were assessed by the organization in charge of the PAR-Q+, then a final Brazilian version was approved. A total of 493 Brazilians between 5 and 93 yr (39.9 ± 25.4 yr), 59% female, with varying levels of health and physical activity, completed the questionnaire twice, in person or online, 1–2 weeks apart.
